Become A Client
Beauty Bus Foundation serves clients with the following diseases and disorders:
- ALS (Lou Gehrig's Disease)
- Multiple Sclerosis
- Muscular Dystrophy
- Parkinson's
- Spinal Cord Injury
- Stroke
- Cancer
... AND their primary caregivers.
Most of our clients are referred by Social Service Agencies or doctors, nurses and hospitals.
To become a Beauty Bus Client a Client Caregiver Application (PDF) must be filled out.
We also need a Doctor Release Form (PDF) completed by the potential client's physician to verify their ability to receive our services.
In addition, client's must also complete a HIPAA Authorization Form (PDF).
Once this is done, the client will receive a call from Beauty Bus to set up an appointment.
Clients may receive 4 complimentary visits per 12 month period. To receive more than 4 services, Beauty Bus asks that the client make a personally meaningful donation to the organization so that we can continue to serve those in need.
